Baby boy is 4 weeks old. We are breast feeding and supplementing with formula. He does great at the breast and doesn’t seem to have any issues latching. The problem is when we give him a bottle. He spills milk out of both sides of his mouth so bad it will saturate two paper towels. He is easily losing an ounce with each feeding. He also acts very overwhelmed by the milk. He will start the feeds off calm with hands in fists and as the feed progresses he has hands open and gasping like drowning.
He does drink very fast so we have tried the paced feeding method, haven’t noticed a huge difference with the milk spilling. We are currently using como tomo bottles with the slow flow (one hole) nipple. Any suggestions on how to reduce the milk spilling from his mouth? Should I change his bottle or nipple?

Make sure he actually latches on t bottle if you hear a clicking when he eats he isn’t latched you might also want to try different bottles with smaller nipples hat helped my son a ton

I find if my daughter is sitting upright too much it will spill out the sides of her mouth if I lay her back more she's fine.

Our son has been exclusively bottle fed. Around 3 weeks, he started having a ton of milk spill out of his mouth while eating after being a perfectly good eater. He just started getting better at it again. Almost 5 weeks. Still some dripping, but not as bad.

I’d try a different bottle. Maybe como is too fast even at level one. Dr browns have been best for us. We’ve tried 4 different brands and just yesterday I put two brands away for later. Lactation consultant said dr browns and platex ventaire are the slowest.

Baby needs breaks every couple minutes. As soon as I see milk on the side I know we need a lil break

I would try Dr Browns bottles with a slower nipple. Try a level 1 or maybe even a premie nipple. When feeding your baby watch for his cues that he needs a break, such as eye brow furrowing, tugging his chin down, widening eyes. These are all signs that the milk is coming too fast and that’s baby’s way of saying “hey mom, slow down!”

my dd does this too the only nipple I have found that eliminated it are the Dr browns wide neck size 1 nipples any other size 1 nipple and it spills out

We have similar issue with comotomo 1 nipple. We did a youtube search for paced bottle feeding that helped and use a bib. I also try to use the AVENT bottle which seems to be less messy. Def just use a little at a time, let the formula float towards the nipple and back again out of it. The video really helped and we burp more often because of the way she is fed. Hope that helps!
